arXiv:astro-ph/9806228 · doi:10.1046/j.1365-8711.2001.03975.x
Abstract
This paper has been withdrawn because the models on which it was based have undergone significant changes and improvements. A new paper with the same title, based on the improved models, is accepted for publication in MNRAS and is available at astro-ph/0006364. We have withdrawn this paper to avoid confusion.
